Notice 1. The adopting parent or parents must file this form with the court before the finalization hearing or order of the court. Within 30 days of the adopting parent or parents receiving a file-marked copy of this agreement, the adopting parent or parents must provide a file marked copy to each person who signed the agreement as well as any licensed adopting agency that placed the child for adoption or consented to the adoption. 2. After the judge signs the Adoption Order for this child, the adoption is final.
