1 Court Findings
The court finds that the restrained person/defendant (name):
a. Is required to carry a specific firearm or ammunition during scheduled work hours as a condition of their 
continued employment. 
b. Cannot be reassigned by their employer to another position where having a firearm or ammunition is not 
needed.
c. Is not otherwise prohibited from having firearms or ammunition under state or federal law.
d. Does not pose an additional threat of harm to any protected person or the public by having access to a specific 
firearm or ammunition.
e. Is a sworn peace officer and:
(1) The court ordered and reviewed a completed psychological evaluation of the restrained person by a 
licensed mental health professional with domestic violence expertise; and
(2) The personal safety of the restrained person depends on their ability to carry a specific firearm or 
ammunition outside of scheduled work hours. 
f. Is not a sworn peace officer and (check (1) or (2)):
The court did not order the restrained person to complete a psychological evaluation.(1)
(2) The court ordered and reviewed a completed psychological evaluation of the restrained person by a 
licensed mental health professional with domestic violence expertise.
