confidential, you can use this form to let the court know if you agree or disagree 
with the request. 
 
Who should use this form? 
You should use this form if you are a minor, parent, or legal guardian who made 
a request to keep information confidential.  
 
What do I need to complete and file this form? 
     You will need three documents that you should have received with this form: 
 
Protected party

Form DV-176, Request for Release of Minor's Confidential Information; 
Form DV-175, Cover Sheet for Confidential Information. 
Form DV-177, Notice of Request for Release of Minor's Confidential Information; and 
You will need to give the court form DV-175 and two copies of your completed form DV-178. Make 
sure you take these forms to the court for filing by the deadline listed on form DV-177.
