PROCEDURES FOR OBTAINING WORK PERMIT 
取得工作許可證的程序  
1. Complete the information required below

3.  For minors
 15 days through kindergarten, please attach a cer tified  copy of minor’s birth certificate.  See reverse side for other 
documents that may be accepted.

4.  Mail
 or present the completed  application to any office  of the  Division of Labor Standards Enforcement for  issuance of your  work 
permit.  Work  permits  will be  issued  within 3 business days and mailed to you.

完整填寫申請表正面要求的所有資訊。

對非學齡兒童的要求 下面的其中一項﹕  
1.) 經過證明的出生證書 
2.) 受洗證書 
3.) 出生醫院的公函 
4.) 護照
學校開學期間， 必須在本學年由授權的學校職員完整填寫申請表，並且注明填寫日期。
學校休學期間， (比如，假期、節日)，要求要麼使用未成年孩童的最新成績報告卡，要麼使用由學校校
長使用學校的信箋寫的信，說明未成年孩童在所有學習課程、健康和出勤方面均”令人滿意”。
如果未成年孩童來自外州，要求要麼使用未成年孩童的最新成績報告卡，要麼使用由學校校長使用學校
的信箋寫的信，說明未成年孩童在所有學習課程、健康和出勤方面均”令人滿意”。
根據成績報告卡簽發的娛樂業工作許可證在學校休學期間有效。請隨申請書附上一個寫好回郵地址、貼
好郵票的信封，以便能夠把許可證郵寄給您。 
* 	 不足一個月的嬰兒必須有經過加州理事會證明在小兒科執業的持照醫生的證明，證明嬰兒至少 15 天
大了，足月生產，並且身體方面能夠經受拍攝電視或電影的壓力。
續期 
1.) 完整填寫的申請表 (全部問題)。 
2.) 隨申請表附上舊許可證副本。 
3.) 附上一個寫好回郵地址，貼好郵票的信封，以便把許可證寄給您。 
4.) 如果學校休學，閱讀上述指示。
如果學校開學，由學校職員填寫申請表的 “學校記錄” 部分，並且簽字。
