before the above-entitled court, where it shall be heard de novo in accordance with Labor Code Section 98.2, and that the Clerk of the Court give Notice 
of time, date and place of the new trial to each of the following parties and the Labor Commissioner’s office at the places listed below.  Appellant certifies 
that a copy of this Notice of Appeal has been served upon the Labor Commissioner and a copy has been mailed to the Respondent, as shown below. 
고소인은 증거물 1 호로써 항소된 명령 , 결정 또는 판정의 복사본을 첨부하였고 , 법원 서기가 노동법 98 조 2 항에 의거하여 이 사건이 처음부터 다시 심리 
될 공판이 지정된 법정에서 이행되도록 조치할 것과 , 법원 서기가 아래에 적힌 소송 당사자들과 노동청 사무실에 새로운 재판의 날짜 , 시간, 장소를 통지 
할 것을 요구합니다 . 고소인은 이 항소 통지서가 노동청장에게 송달되었고 그 복사본이 아래에 명시된 피고인에게 우편에 의해 보내졌다는 사실을 
보증합니다 .
